Please arrive 15 minutes before your appointment time. This will allow us to check all your paperwork so you can get the most out of your time with your dental team. If you’re running late, just give us a call to let us know.

While completing and submitting your paperwork online allows us to optimize your experience, if you do need to fill out paperwork in the office we are happy to accommodate you. Please arrive 10–15 minutes before your appointment time to fill out your paperwork and help us keep you on schedule.
