We’ll gather your comfort preferences, health history, and send insurance/payment paperwork so you can maximize your one-on-one time.

Get ready for your appointment

Complete your forms and bring them along with your insurance card and any questions.

Before you arrive

Complete forms online if possible and plan to arrive 10–15 minutes early so we can keep you on schedule.

  • Print and sign any reminders/consent forms you’ve received.

During your visit

FAQs

  • Please arrive 15 minutes before your appointment time. This will allow us to check all your paperwork so you can get the most out of your time with your dental team. If you’re running late, just give us a call to let us know.

  • While completing and submitting your paperwork online allows us to optimize your experience, if you do need to fill out paperwork in the office we are happy to accommodate you. Please arrive 10–15 minutes before your appointment time to fill out your paperwork and help us keep you on schedule.

  • If you are unable to make your appointment, please let us know at least 48 hours before your appointment so we can offer the time slot to another patient.
